The idea that he could potentially face people who previously showed him kindness makes him conflicted. At the same time, he knows that he has the ability to save people now. That it's not all lost. The sense of paranoia is suffocating, especially when he has all those flashbacks to how badly distrust ended for him and for everyone in the past life.

He wishes he could clear his head and just focus.

But maybe eventually the sense of duty wins, because if you didn't encounter him sulking, then you may also see Ruca in a quite opposite mood. When he manages to heal someone, he can be seen outside even after the dark. He seems to be strangely confident and completely blind to any danger, just venturing onwards in search of more opportunities to be useful.
